What health care driver was the policy designed to address: cost, quality, access, or a combination?

Health Care Policy

Health Care Policy

In this week’s media presentation, Dr. Trautman, Dr. Wakefield, and Ms. Coyle discuss how public policies initiated at the national or state level ultimately influence what occurs at the institutional and local level of nursing practice and health care delivery. Health care policy usually is developed to address health care cost, quality, or access, or a combination of the three. Due to the nature of their interaction with patients, nurses are well situated to be effective, knowledgeable advocates for their patients.

In this week’s Discussion, you will examine how current policies impact how you as a nurse provide health care. You will then select an issue of interest and determine how you could advocate for policy in that area.

To prepare:

  • Select one public policy that currently is impacting you and your practice. Consider the following:
    • What health care driver was the policy designed to address: cost, quality, access, or a combination?
    • Does the policy appear to be achieving its intended results? On what data are you basing your assumption?
    • What have been the effects (adverse or positive) of this policy on health care cost, quality, and access?
    • How is this policy affecting your nursing practice?
  • Next, select a health care issue—something you see or experience on a daily basis—about which you would like to influence a practice change through the policy process.
  • Review the Learning Resources, focusing on Kingdon’s Model. How would you utilize this model to guide your policy development?

Post by Day 3 a brief description of a public policy that is having an impact on your practice; summarize your analysis of the policy using the bulleted list above as a guide. Then, summarize the issue you would like to influence though a change in public policy, and outline how you would utilize Kingdon’s Model to guide your policy development.

Discuss how you would conduct an admission-seeking interview of the plant controller.

Please answer 3 of the following questions.

As the director of the internal auditing and fraud investigation departments for a large retail company that buys and sells large volumes of products, you have received an anonymous tip from a confidential employee in one of the high-tech warehouses. The confidential employee noted that there was a major embezzlement taking place in one of the divisions. Members of the internal audit department had completed a routine review of the internal control of that division the previous year, and they found that the control system was properly designed and operating effectively. Only minor recommendations were made, and the next review is due 3 years later.

The confidential employee noted the erratic behavior and lavish lifestyle of the warehouse controller and stated that the controller has been overriding existing controls, which would prevent any audit from being able to detect the embezzlement. The warehouse controller is well-respected and highly trusted by the chief executive officer (CEO) and the chief financial officer (CFO), and their families are involved in the same civic organizations. Investigating the plant controller could cause considerable disruption in the company and the personal lives of the employees.

Please answer 3 of the following questions. As you answer the questions, you must provide support or evidence that will enhance and empirically prove your answers. Academic criminal justice articles or real life criminal justice findings that are not found in journals or other academic sources must be used in supporting your answers. Please use APA style for all cited sources, including your resource page.

  • Given the situation, discuss some of the options that the company has for handling this situation.
  • Assuming you decide to investigate these allegations, describe the investigation steps.
  • In preparation for the interviews, note the order of the witnesses who would be interviewed and how you would plan, conduct, and document your interviews.
  • Discuss how you would conduct an admission-seeking interview of the plant controller.

Week 7: Cognitive and Behavioral Applications

Introduction

As you think about the essential components of cognitive and behavioral theories, what clients do you envision as best suited for these approaches? If you were a client in one of the videos you have explored thus far, how might you react to behavioral or cognitive interventions? On the other hand, if you were the counselor in one of the videos, how comfortable would you be employing a cognitive or behavioral approach?

In previous Application weeks, you have had the opportunity to observe humanistic, existential, and psychoanalytic theories in action. This week you increase your acumen in cognitive and behavioral theory (CBT) applications by watching media demonstrations of CBT counselors. Remember that all media pieces are 90–120 minutes in length. Be sure to select your media piece early in the week and plan your time accordingly.

Objectives

Students will:

  • Analyze theoretical approaches in counseling sessions
  • Evaluate the effectiveness of counselors’ theoretical approach
  • Analyze cognitive and behavioral theories
  • Evaluate the effectiveness of cognitive and behavioral theoretical approaches in working with a specific client population

Please proceed to the Resources.

Week 7 Discussion

Theory in Action: Cognitive and Behavioral Approaches

Have you ever heard the term “positive addiction”? In general, this term is used to describe the act of replacing a negative addiction, like smoking, with a positive activity, like exercising. Both smoking and exercising can be perceived as a means to reduce stress; however the act of exercising is clearly the healthier option of the two. William Glasser, the founder of reality therapy was the first to introduce this term. Glasser believed that these positive addictions can increase the strength and motivation that people feel in their lives.

Have you ever stood on a crowded elevator and considered singing a song? Did you know that this could actually be an intervention used by a cognitive-behavioral counselor called a shame-attacking exercise? In shame-attacking exercises, clients engage in an activity that others around them might disapprove of, thus causing shame to the client. Experiencing the brief discomfort caused by these actions is thought to decrease feelings of social anxiety and self-consciousness.

For this week’s Discussion, you consider which theoretical approach most resonates with you: reality therapy or a generalized CBT approach. You then watch the media piece that translates this theory into practice. Specifically, you examine how the counselor depicted in the media used your selected theoretical approach to guide his or her interventions.

Note: When crafting your Discussion post, support your ideas by providing references to specific examples from the video. In addition, cite the video frame (e.g., 8.32 minutes) that the action or intervention occurred. You do not need to summarize the demonstration itself, as your Instructor and many of your colleagues have also viewed it. Instead, provide insight into the interventions used and major constructs of the theory.

Post by Day 3 a brief description of the video you selected and the theoretical approach used. Explain how this theory resonates with you. Then, describe the counselor’s specific interventions that would indicate his/her theoretical approach and explain how effectively the counselor demonstrated the approach. Finally, share one insight you gained from watching the counselor in the media.

Week 8: Family, Feminist, and Transpersonal Theories

Introduction

Humans naturally view the world through their own eyes. Their opinions, assumptions, and reactions hinge on how they personally perceive situations. In fact, it can be argued that humans are fairly self-obsessed. As a counselor, it is imperative to be cognizant of this natural tendency, and take strides to view the world through the eyes of others.

Postmodernism, a theoretical approach that is largely influenced by counselors’ ability to think like their clients, prompts counselors to enter their clients’ realities. From this vantage point, counselors can use the narratives of their clients to travel through perceptions of problems and life events. Side-by-side in this reality, counselors can work with clients to restructure their life-stories.

As a counselor, role-playing is one of the first activities in which you can engage to place yourself in clients’ reality. Role-playing allows you to experience the mixture of feelings that clients may experience when in the counseling environment. This week, you use the media to role-play three first-time visits with counselors from different postmodern theoretical approaches.

Objectives

Students will:

  • Evaluate effectiveness of postmodern theories

  • Apply knowledge of postmodern theories

Postmodern Theories Program Transcript Instructions: Watch the following introductions by the various counselors.

Post by Day 3 an explanation of which approach might be most effective and explain why. Then explain how this particular approach may help a client to achieve his or her goals. Finally, explain what it felt like to sit in the client’s seat.

Family Theory Counselor:

Hello, I’m Bob. It’s nice to meet you.

I’m a National Certified Counselor and a Licensed Professional Counselor in my state.

In my work with clients, I use a family therapy approach. Often when one individual in a family has a problem, it’s related to a problem in the family.

As we talk today, I may suggest that in the future you come in with your partner or your entire family to determine if there is a stressor that is not only affecting you but the entire family.

For example, sometimes if a couple is having marriage problems, one of the children begins misbehaving to take the focus off of the parents. The parents focus on the child which leads to a break in the arguments.

This is called triangulation. I usually ask my clients to develop a genogram or eco-map to help us better understand family development and how families interact with their communities.

If you like, I can tell you more about my specific family theory; however, what’s important to know now is that as a family counselor, I can work with individuals, couples, and/or families, although I generally work with couples or families.

Feminist Theory Counselor:

Hello, I’m Sue. It’s nice to meet you.

I’m a National Certified Counselor and a Licensed Professional Counselor in my state.

In my work with clients, I use a feminist theory approach.

You might think that means that I only work with women, but that’s actually not true. I work with both men and women; in fact, there are a few male counselors at my center who also integrate a feminist theory approach into their work with their clients.

As a feminist counselor, I’ll consider myself to be a partner with you in reaching your goals. I want to understand your experiences and views and the environment in which you have lived. I’ll be less interested in diagnosing you than in working with you to determine how you have been affected by any type of oppression or marginalization. I draw from a variety of approaches, depending on what works best for you.

For example, I’ve found bibliotherapy and assertiveness training to be helpful for many clients. If you have any other questions, feel free to ask me at any time.

Transpersonal Theory Counselor

Hello, I’m Cynthia. It’s nice to meet you.

I’m a National Certified Counselor and a Licensed Professional Counselor in my state.

In my work with clients, I use a transpersonal approach.

With a transpersonal approach, I want to work with you to help you overcome your difficulties so that you can ultimately focus on your spiritual development.

I draw from a variety of both Eastern and Western approaches in helping my clients overcome difficulties that may affect their potential.

For example, many of my clients have found meditation, mindfulness, yoga, and cognitive-behavioral interventions to be helpful for coping with anxiety which can ultimately block spiritual development.

Those are just a few examples of how I integrate a variety of approaches in my work with clients.

The main difference you’ll find in working with me compared to other counselors is that I believe there are stages of development beyond the ego.

                                             Week 8 Discussion

Applying Family, Feminist, and Transpersonal Theories

Reflect upon two personal or professional goals you might like to achieve. If you were to visit a counselor with these goals in mind, which theoretical approach might help you to achieve them? In addition, what type of counseling style might you connect with the most?

In this Discussion you use the media, Postmodern Theories, to seek the help of three counselors. As you listen to each counselor’s theoretical approach, consider how each might be the most effective in addressing your unique goals. In addition, take note of the emotions that you feel while you virtually sit in the client’s seat.

Note: This post does not require you to share any personal information about yourself. Though you should use personal or professional goals to assist you in selecting a theoretical orientation, you may present the ideas of your posting in a more general format.

Post by Day 3 an explanation of which approach might be most effective and explain why. Then, explain how this particular approach may help a client to achieve his or her goals. Finally, explain what it felt like to sit in the client’s seat.

Focus of the Final Exam

The purpose of the Final Exam is to assess your understanding of the main statistical concepts covered in this course and to evaluate your ability to critically review a quantitative research article.  The exam will consist of two parts: Part I includes three essay questions and Part II includes a research critique.  All of your responses should be included in a single Word document for submission.

Please include the following general headings for each section of the written exam within your Word document:

Part I: Essay Questions

  1. Essay 1
  2. Essay 2
  3. Essay 3

Part II: Research Study Critique

  1.   Introduction
  2.   Methods
  3.   Results
  4.   Discussion

Your complete Word document must include a title page with the following:

  1. Student’s name
  2. Course name and number
  3. Instructor’s name
  4. Date submitted

Part I: Essay Questions

There are three essay questions in this section.  You must answer all three questions.  The length of each essay should be one to two double-spaced pages (excluding title and reference pages).  Use 12-point font and format your paper with regular 1-inch margins.  Do not include the essay prompt in your document.  It will not count toward the length requirement for your essays.

Essay 1

A group of researchers conducted an experiment to determine which vaccine is more effective for preventing getting the flu. They tested two different types of vaccines: a shot and a nasal spray. To test the effectiveness, 1000 participants were randomly selected with 500 people getting the shot and 500 the nasal spray. Of the 500 people were treated with the shot, 80 developed the flu and 420 did not. Of the people who were treated with the nasal spray, 120 people developed the flu and 380 did not. The level of significance was set at .05. The proportion of people who were treated with the shot who developed the flu = .16, and the proportion of the people who were treated with the nasal spray was .24. The calculated p value = .0008.

For this essay, describe the statistical approaches (e.g., identify the hypotheses and research methods) used in this excerpt from a research study.  Interpret the statistical results and examine the limitations of the statistical methods.  Finally, evaluate the research study as a whole and apply what you have learned about hypothesis testing and inferential statistics by discussing how you might conduct a follow-up study.

Your essay must address the following points:

  • Describe the research question for this experiment.
    • What were the null and alternative hypotheses?
    • Were the results of this test statistically significant?
    • If so, why were they significant?
  • Would the researchers reject or fail to reject the null hypothesis?
  • Do the results provide sufficient evidence to support the alternative hypothesis?
  • Was the sample appropriate for this study?  Explain your answer.
  • What are some possible limitations to this study?
  • Discuss how you would conduct a follow up study to this one. Explain your answer.
  • Describe the difference between practical and statistical significance.

Essay 2

A researcher has investigated the relationship between IQ and grade point average (GPA) and found the correlation to be .75.

For this essay, critique the results and interpretation of a correlational study.

  • Evaluate the correlational result and identify the strength of the correlation.
  •  Examine the assumptions and limitations of the possible connection between the researcher’s chosen variables.
  • Identify and describe other statistical tests that could be used to study this relationship.

Your essay response must address the following questions:

  • How strong is this correlation?
    • Is this a positive or negative correlation?
    • What does this correlation mean?
  • Does this correlation imply that individuals with high Intelligence Quotients (IQ) have high Grade Point Averages (GPA)?
  • Does this correlation provide evidence that high IQ causes GPA to go higher?
    • What other variables might be influencing this relationship?
  • What is the connection between correlation and causation?
  • What are some of the factors that affect the size of this correlation?
  • Is correlation a good test for predicting GPA?
    • If not, what statistical tests should a researcher use, and why?

Essay 3

A researcher has recorded the reaction times of 20 individuals on a memory assessment. The following table indicates the individual times:

In this essay, demonstrate your ability to organize data into meaningful sets, calculate basic descriptive statistics, interpret the results, and evaluate the effects of outliers and changes in the variables.  You may use Excel, one of the many free online descriptive statistics calculators, or calculate the values by hand and/or with a calculator.

Next, separate the data into two groups of 10; one group will be the lower reaction times, and the second group will be the higher reaction times.  Then, address the following points in your essay response:

  • Calculate the sum, mean, mode, median, standard deviation, range, skew, and kurtosis for each group.
  • How do the two groups differ?
  • Are there any outliers in either data group?
  • What effect does an outlier have on a sample?

Lastly, double each sample by repeating the same 10 data points in each group.  You will have a total of 20 data points for each group. After completing this, address the following in your essay response:

  • Calculate the following for the new data groups: sum, mean, mode, median, standard deviation, range, skew, and kurtosis.
  • Did any of the values change?
  • How does sample size affect those values?

Part B: Research Study Critique

In this second portion of the Final Exam, you will identify and critically evaluate a quantitative research article based on a social science topic.  Your selected article must include a research question(s) and/or hypothesis(es) and utilize statistical analyses covered in the course. The article must be peer-reviewed and published within the last 10 years.

In the body of your critique, describe the statistical approaches used, the variables included, the hypothesis(es) proposed, and the interpretation of the results.   In your conclusion, suggest other statistical approaches that could have been used and, if appropriate, suggest alternative interpretations of the results.  This process will allow you to apply the concepts learned throughout the course in the interpretation of actual scientific research. Your critique must include the following sections:

  1. Introduction: This section will include a general introduction of the quantitative study from a peer-reviewed source published within the last 10 years.  The research questions and/or hypothesis(es) as well as the purpose of the study should be clearly defined.
  2. Methods: Describe and evaluate the procedures and methods of data collection, measures/instruments used, the participants and how they were selected, and the statistical techniques used.
  3. Results: In this section describe and critique the results presented in the study.
  4. Discussion: Discuss and evaluate the efficacy of the results presented in the study.  Address, the strengths, weaknesses, and limitations of the study, and suggest future research directions.  Include additional forms of statistical analyses as part of the suggestions for future research.

The Research Study Critique:

  1. Must be three to four double-spaced pages in length (excluding title and reference pages) and formatted according to APA style as outlined in the Ashford Writing Center.  Use 12-point font, with 1-inch margins.
  2. Must use the sections and headings described above.
  3. Must address the article with critical thought by examining, reflecting, and evaluating the article from an objective viewpoint and by using facts to support your argument. Refer to the Critical Thinking Community website for further assistance.
  4. Must end with a conclusion that summarizes your critical evaluation.
  5. Must use one quantitative research study from a peer-reviewed source that was published within the last 10 years.
  6. Must properly cite the source article in APA style as outlined in the Ashford Writing Center.
  7. Must include a separate reference page, formatted according to APA style as outlined in the Ashford Writing Center.

With each element, ask yourself, would I really incorporate this element in real life?

Business Plan

Business Plan or Business Proposal

Bottomline: PLEASE READ and see attachments!!!

Although you will each be working on separate projects, depending on your previous coursework, you will come together in the discussions to share your ideas from your various perspectives. The business world operates through the use of cross-functional teams and understanding the similarities and differences in perspectives is essential to your future success. With this in mind, the course is organized into subfolders related to each of your degree programs. You will notice that the folders are grouped into two primary areas, depending on your degree program, which will drive the type of project you will be doing.

Whether you are working on a business plan or a project proposal, your work will be presented unlike typical assignments you have had in your program. You will be moving away from a researched based essay to actually presenting your material for consideration. Think of yourself as a business leader trying to gain support from shareholders, or a business leader who already has the support, and is trying to gain new clients. You can also be a new entrepreneur who wants to diversify and gain financial backing. With each element, ask yourself, would I really incorporate this element in real life? If not, is there a way to customize the material to make it work? The work will require you to critically analyze your work.

The same goes for a project proposal. You are a project leader trying to gain support of upper management to implement a project solution. You can also be a project leader who already has support and are working through the elements of adopting the project. I always tell my students, you are working within the guidelines of the course assignments but you are transforming the information that will support the effort that allows you to present to the correct audience.  So, when the assignment calls you to write an Executive Summary (you may instead use an Abstract), remember you are not describing why an ES is important or what elements should be included in the section, you are writing one!  You need to have a fundamental understanding that each sentence you write should not “teach or explain” a concept. Your work should always be directed towards what your company does or plans to accomplish.
